Helpful tips before you get started

  • This tool is powered by a bot, not a live agent
  • You’ll be asked for your contact information, as well as the member’s name, date of birth and Ontario Teachers’ account number
  • The more information you include, the faster it is to match your files to the correct account so provide as much detail as you can 
  • We’ll review everything you send. If we have questions, we’ll contact you, so please ensure your contact information is accurate 
  • Accepted file types: PDF, JPG, JPEG, PNG, HEIC and HEIF
  • Maximum file size: 10MB per file
  • You can upload up to five files per session
